 Request for Qualifications

The Bluegrass Area Development District will receive request for qualifications to provide services through the following funding sources:  Title IIIB Ombudsman program, Title VII Ombudsman and Elder Rights Protection Activities; Title IIIB Legal Services and State Health Insurance Program; and Adult Day Social Model services. The Bluegrass Area Development District will also receive request for qualifications for the following services in Franklin County:  Title IIIB Supportive Services, C-1 Congregate Meals & C-2 Home Delivered Meals, Homecare Program Direct Service Provider, and Homecare Program Case Management. This project is funded in part, under a contract with the Kentucky Cabinet for Health and Family Services, from the Administration on Aging of the U.S. Department of Health and Family Services.  Application packets will be available for pick up anytime after 8:00am, EST on January 11, 2012 at the Bluegrass ADD office located at 699 Perimeter Dr., Lexington, KY 40517. Completed applications will be due in the Bluegrass ADD office by 4:30pm, EST on February 1, 2012.  No applications will be accepted after this date.
